Clinically the lesions appear as a painless nodular or papular
elevation with a small central crater and smooth or pappillomatous
surface.It is sessile with whitish or normal color and a diameter
ranging from a few millimeter to 1 cm

Histopathology

  • Circumscribed
  • Cup-shaped
  • Invaginating
  • Extends into underlying dermis
    Warty dyskeratoma. A cup-shaped invagination is filled with a keratinous plug, which in turn overlies an area of suprabasal clefting. (H&E)

  • Central depression:
    • filled with plug of keratinous material containing some grains:
      • keratin plugs sometimes dislodged, particularly in oral lesions
  • Epidermal component:
    • suprabasilar clefting:
      • numerous acantholytic and dyskeratotic cells within lacuna
  • Dermal papillae (villi):
    -covered by layer of basal cells
    -protrude into lacuna
    • contain:
    dilated vessels
    occasional melanophages
    few inflammatory cells also present in underlying dermis
  • Pilosebaceous follicles may open into lesion
  • Corps ronds and grains:
    • better developed in skin lesions than in those in mouth
